Please take the time to read instructions and fill out forms completely.
THERE ARE FOUR STEPS!
Please
complete and TURN IN at Parent Meeting on December 1st (6:00 p.m. at Rob Gym)
 MAIL ALL paperwork to THE CLUB (if you cannot turn in at meeting)
MAIL NOTHING to the SCVA

$45 USAV membership fee, the

SCVA ONLINE PLAYER REGISTRATION - Please do ASAP!
The USAV Waiver/Code of Conduct Form and Medical History Form must be printed, completed, and included with the documents located below for all Travel players!

(If you participated on a travel team last season AND remember your login and password, you can simply update your information & print forms - if you cannot remember your password, click on "forgot password"
if you cannot remember the email address you used last year, you must use "NEW MEMBER")

DOWNLOAD, PRINT, AND SIGN THE FOLLOWING CLUB DOCUMENTS
SBVC/MEMBER AGREEMENT CONTRACT
SBVC ACKNOWLEDGMENT
SBVC WAIVER
PLAYER PROFILE - Juniors & Seniors Only
Please fill in the appropriate responses, save to your desktop, then attach it with an email to sbvbclub@west.net - The Club will take pictures and include in profile, and assign jersey numbers, when practices begin.

Checklist for Packet Contents:
USAV Waiver/Code of Conduct (from SCVA online registration)
USAV Medical History (from SCVA online registration)
SBVC Club/Member Agreement Contract
SBVC Waiver of Liability
SBVC Acknowledgment (including proof of auto insurance)
Pay Online HERE or make Check out to SBVC (see APPENDIX: 1 of Player/Parent Policy Manual)
You
should have SIX documents total and copy of insurance (and
a check).
