Juniors & Seniors - PLAYER PROFILE
Please fill in the appropriate responses, save to your desktop, then attach it with an email to sbvbclub@west.net - The Club will take pictures and include in profile, and assign jersey numbers, when practices begin.

USAV Waiver/Code of Conduct (from SCVA online registration)
USAV Medical History (from SCVA online registration)
SBVC Club/Member Agreement Contract
SBVC Waiver of Liability
SBVC Acknowledgment (including proof of auto insurance)
SBVC Fundraiser Flyer Form (select Option A or B)
Make Check out to SBVC (see APPENDIX: 1 of Player/Parent Policy Manual)
You
should have 6 documents total and copy of insurance (and
a check).
