STEP 2:	READ PLAYER/PARENT POLICY MANUAL MANUAL
STEP 3:	DOWNLOAD, PRINT, AND SIGN THE FOLLOWING DOCUMENTS SBVC/MEMBER AGREEMENT CONTRACT SBVC ACKNOWLEDGMENT SBVC WAIVER
STEP 4:	MAIL COMPLETED DOCUMENTS TO THE CLUB ASAP! Include First Program Fee Payment Checklist for Mailing: USAV Waiver/Code of Conduct (from SCVA online registration) USAV Medical History (from SCVA online registration) SBVC Club/Member Agreement Contract SBVC Waiver of Liability  SBVC Acknowledgment (including proof of auto insurance) Check made out to SBVC (see APPENDIX: 1 of Player/Parent Policy Manual) You should have FIVE documents total, plus copy of auto insurance and a check.
